ONLINE ATHLETIC PACKET FOR HIGH SCHOOL FALL SPORTS IS NOW OPEN

The online athletic packet for high school fall sports registration will open up on Wednesday, September 2, 2020. The online athletic packet MUST be completed for high school fall sports clearance. The packet can be accessed on the Athletics & Clubs menu on the left side of this page.

Please note that if your child was cleared and participated in the voluntary summer workouts, you do not need to turn in any additional paperwork for fall sports clearance. You will just need to complete the online athletic packet.

If your child was not cleared for voluntary summer workouts, paperwork from the online athletic packet MUST be turned in at collection on Tuesday, September 8, 2020. Collection will take place outside the Hamilton lobby doors from 1 - 2:30 pm.

**Please note that the memo above only pertains to high school fall sports. Paperwork for middle school sports is not being collected at this time.**


IMPORTANT INFORMATION REGARDING
HIGH SCHOOL SUMMER WORKOUTS AND HIGH SCHOOL FALL SPORTS

Recently the NJSIAA notified all schools that the fall season has been pushed back to begin on September 14th.  The information pertaining to the delayed season can be found in the folder "SUMMER PRACTICES" below. Our conference is now in the process of making adjustments to our fall schedules and once the new schedules are completed we will make them available. Since we now have some direction we will begin Phase 1 of the NJSIAA’s plan during the week of August 3rd. During that week Boys Soccer, Football, and Volleyball will begin and we have included those dates and times. During the week of August 10th Girls Soccer and Boys & Girls Cross Country will begin with those dates and times to follow. These workouts are purely voluntary, and a student athlete will not be penalized in any way should they not attend these workouts.

HIGH SCHOOL SUMMER/FALL ATHLETIC FORM COLLECTION

TUESDAY, AUGUST 4TH - 9:15 - 11:15 AM

Please make sure to wear a mask and have all necessary forms completed.
This is the last in-person collection date for summer workouts.

**EMAILS WILL NO LONGER BE ACCEPTED AS A FORM OF SUBMITTING PAPERWORK**

*Even with the pending legislation on extending expired physicals for the fall 2020 season, athletes will still need to turn in the Health History Update Questionnaire along with any asthma or allergy forms and other COVID-19 related documentation. This legislation has not been signed into law by Governor Murphy as of 7/31/20.*

If you would like your child (ren) to participate in voluntary summer workouts, you MUST read the document "Clearance for High School Summer Workouts" located on this page regarding the forms that are required.
  • Students will not be allowed to practice until proof of a physical within 365 days prior to the first official starting day of that particular sport season, the online athletic packet has been completed, the confirmation slip has been submitted to the athletic trainer, and the school physician has reviewed and cleared the student to participate.
  • The pre-participation physical evaluation form must be filled out completely. Incomplete forms will not be accepted.
  • If a student has an allergy or asthma, the proper documentation must be submitted with the pre-participation physical evaluation form in order to be eligible for medical clearance. These forms can be found on the athletics’ website or the student health services website.

Download the app and after opening choose 'New Jersey', then choose 'New Jersey Interscholastic Conference', and then choose Glen Rock!


INJURIES

  • All injuries must be reported to the coach, athletic trainer, and school nurse.
  • If an athlete visits with a physician for an injury, the athlete must provide medical documentation to the athletic trainer and school nurse. This documentation shall include the diagnosis, limitations and or recommendations, and include a return to play date. A referral form can be found on the athletics’ website to satisfy all requirements.
  • An athlete will not be cleared to return to activity until medical documentation has been received.
  • An athlete who is returning from an injury or an illness is required to participate in one practice before he or she competes interscholastically.

CONCUSSIONS

  • All athletes must take IMPACT before they can participate in athletics.
  • Athletes are IMPACT tested every two years.
  • If an athlete sustains a concussion, he or she will need to visit with a physician who has been trained in the evaluation and management of concussions. A concussion clearance form is required to be completed and returned to the athletic trainer. This form can be found on the athletics’ website.
  • Athletes who have been diagnosed as having a concussion must see the athletic trainer on a daily basis to fill out the post-concussion signs and symptoms check-list.
  • All athletes who sustain a concussion must be asymptomatic for seven consecutive days before completing the five day return to play protocol.

INSURANCE COVERAGE

  • Athletes injured at practice or a game will immediately notify the coach, the athletic trainer, and school nurse.
  • The coach and the athletic trainer will complete an accident report within 48 hours of the injury. This report will be filed in the nurse’s office.
  • The athletic trainer will issue the athletic insurance claim form only upon the parent/guardian’s request.
  • Interscholastic sports insurance covers all athletes; this is a rider policy. All bills are first sent to the athlete’s insurance carrier and any remaining costs are then submitted to the interscholastic sports insurance.
  • It is the parent/guardian’s responsibility to submit all bills to the insurance companies.
