Medication

Please note that no medication - including both prescription and over-the-counter (OTC) medications - will be administered to students unless there is a Physician and Parent/Guardian medication authorization form completed (found below). This policy also applies to inhalers for asthma and epipens for allergies. Download forms as needed.

Completed forms, along with the prescribed medication in a prescription-labeled package
 or OTC medication in its original OTC container, should be brought to the Health Office at the beginning of each school year. Note: Any student participating in sports who requires an asthma inhaler or epipen will not be cleared until the forms are submitted.
